I also have a whistle attached to my PFD.
I opened the PFD and inflated it manually with the mouthpiece. I did that to make doubly sure I wasn't puncturing the inflatable bladder. Then I cut a small slice through the fabric up near my shoulder. My whistle is on a one-foot piece of heavy cord with a large button on the end. The button goes through the slit I cut, and the corded whistle rides inside the closed PFD out of the way. No tangle, no muss - but with the PFD inflated, it's right there.
