If you live in the Ogden area, Wangsgaards (at Five Points) has crawlers, wax worms, meal worms and even spikes in stock. Inexpensive, too!

The standard little plastic tub, with 25 waxies for $2. Wangsgaards sells crawlers for a buck a dozen, which is the lowest I've found. I buy new bait for every trip, and those are just about perfect amounts for me.

Smith and Edwards has 250 count wax worms for $6.99. They just got their shipment in on Thurs. this week! If you keep them cool but not frozen, they will last a long time, I've had them last until Mar.
